AN INVESTIGATION is under way into equipment which may have led to the death of a patient at Liverpool’s Royal hospital. The family has sought legal advice after a medical device failed during treatment. It has been sent to the manufacturer for inspection and the matter is being investigated by Liverpool coroner Andre Rebello and theŠMedicines and Healthcare products Regulatory Agency (MHRA).
