An elderly widow died after a nurse mixed her up with another patient and gave her a fatal dose of the wrong medication. Sylvia Roberts, 88, was the victim of a 'thoughtless and cavalier' approach by hospital staff who made a horrifying series of errors, a coroner ruled.
