There's a new card in my purse. It can't buy anything and is worthless to a thief. But it is potentially the most valuable item I possess: it could determine whether I live or die. 'Stop!' it declares. 'I have made an advance decision to refuse treatment.' The card tells medical staff that I have made a legally enforceable decision not to have specified medical treatments, just in case I'm not able to tell them myself.
