The government stepped up its battle with the British Medical Association yesterday over plans to force GPs to extend surgery opening hours. Alan Johnson, the health secretary, bypassed the BMA leadership by writing directly to every GP in England, appealing for cooperation in a scheme to provide evening and weekend appointments.
