A man who stabbed to death a bus passenger as he attempted to defend his girlfriend had been wrongly released from jail only hours earlier following a series of official blunders, it emerged yesterday. The Government last night launched an investigation after admitting catastrophic "failings" throughout the criminal justice system had allowed Anthony Joseph to kill a man when he should have been in custody.
